Participation Stipulation

For participant’s own well-being, we remind you that the Dublin Community Senior Center is not responsible for providing care for individuals who have a physical or mental impairment that causes an inability to care for their own personal needs, or that may jeopardize the rights of other members of the group.

 Membership (Participation/Emergency) Form

All seniors who attend any programs, classes, events, etc. at the Dublin Community Senior Center are required to complete a member participation/emergency form for the current year. Every January of a new year, we do ask ask all members complete a new form to stay current in our directory and in case of an emergency.
